Provisional Improvement Notice (PIN) Builder

A trained HSR may issue a Provisional Improvement Notice stating their belief, the specific provision contravened, briefly how, and a day to remedy that is at least 8 days after issue (model WHS Act ss90-101). The recipient may seek an inspector review within 7 days (Queensland 3 days; Victoria's OHS Act differs).

Only a trained HSR may issue a PIN

Only a Health and Safety Representative who has completed the approved HSR training course may issue a Provisional Improvement Notice (s90; an untrained HSR must not issue a PIN under s71). Confirm your training is current before issuing this notice.

The notice

A PIN must state your belief, the specific provision contravened, and briefly how it is being contravened (s91).

Dates

The day to remedy the contravention must be at least 8 days after the notice is issued (s91).

Complete the issued-to, your name, the provision, how it is being contravened, and a remedy-by date at least 8 days after issue to build the notice.
